"Accident" may describe the unexpected, unintentional nature of most collisions, but is worthless for describing what actually happened. Distraction, intoxication, carelessness, mechanical failure (a truck tire blowing next to you and taking you out), landslide, flood, weather, visibility, traction failure, on and on the circumstances go...and it's the circumstances that describe the collision. And many of these circumstances are in no way an accident. Collision is a much better word.

You're conflating payer with provider. My health insurance is through Medicare. In over three years I have never been disappointed with the healthcare provided. The county population is about 200,000, with no urban center within hundreds of miles. Yet I can choose from a very broad array of providers in my area. The market for Medicare insured patients is too large for providers to not participate. With Medicare, single payer is not a single choice.
