Yes, the main problem is that neither the LLMs nor most people are self aware - self aware people, even mentally ill ones, can take encouragement where it makes sense and ignore it or even rebuke the LLM where it doesn't.
But at least the LLMs are getting better at this - ChatGPT o3 is not afraid to critisize if it thinks the user has a bad idea. It can be persuaded that the idea is nevertheless reasonable due to some constraints, but that's about it. It is also generally far less friendly and doesn't try to please.
